Criminal Record Check (North Perth Outlaws Lacrosse)

PrintCriminal Record Check

NEW VOLUNTEERS

All lacrosse volunteers in Ontario are required to provide a Criminal Record Check (Vulnerable Sector).  This applies to Executive Members, Coaches, Assistant Coaches, Trainers and Managers.  It must be completed prior to commencing their initial volunteer assignment.

Interested volunteers are required to submit a copy of the “Criminal Record Check” letter in our NPLA Library to a local OPP station to support their application.  The process takes approximately 2-3 weeks.  North Perth Lacrosse has a relationship with the OPP station in Listowel to perform these checks in the most economical manner.

RETURNING VOLUNTEERS

In subsequent years, volunteers are required to complete an Offence Declaration form which lists all Criminal Convictions since the date of the last Criminal Records Check.  This also applies to Executive Members, Coaches, Assistant Coaches, Trainers and Managers.  It must be completed prior to commencing any subsequent volunteer assignments.

Returning volunteers are required to submit a completed copy of the “OLA Offense Declaration Form” in our NPLA Library to the Director of Player Development prior to the start of assuming their volunteer role.
